I have nothing much against Hitomi, it was just a reeeeeeaaaaaaally inopportune time for her to say that.
Really? Sayaka had gone to the hospital all those times, and she had all those chances to tell him. Once he came back to school, Hitomi guessed she could confess to him (since she probably didn't know where he was in the hospital?) but she knew Sayaka liked him, so she gave her the opportunity. But then you have to blame Sayaka for not confessing, after all, she had all those other times beforehand to also tell him.
